چکیده

This study describes the long-term effectiveness on spatial neglect recovery of a 2-week treatment based on prism adaptation (PA). Seven right-brain-damaged patients affected by chronic neglect were evaluated before, after two weeks of the PA treatment and at a follow-up (variable between 8 and 30 months after the end of PA). Neglect evaluation was performed by means of BIT (conventional and behavioral), Fluff Test, and Comb and Razor Test. The results highlight an improvement, after the PA training, in both tasks performed using the hand trained in PA treatment and in behavioral tasks not requiring a manual motor response. Such effects extend, even if not significantly, to all BIT subtests. These results support previous findings, showing that PA improves neglect also on imagery tasks with no manual component, and provide further evidence for long-lasting efficacy of PA training. Dissociations have been found with regard to PA efficacy on peripersonal, personal, and representational neglect, visuospatial agraphia and neglect dyslexia. In particular, we found no significant differences between the pre-training and post-training PA session in personal neglect measures, and a poor recovery of neglect dyslexia after PA treatment. The recruitment of a larger sample could help to confirm the effectiveness of the prismatic lenses with regard to the different clinical manifestations of spatial neglect.

Effects of prism adaptation on motor-intentional spatial bias in neglect.

Prism adaptation may alleviate some symptoms of spatial neglect. However, the mechanism through which this technique works is still unclear. This study investigated whether prism adaptation differentially affects dysfunction in perceptual-attentional 'where' bias versus motor-intentional 'aiming' bias. Five neglect patients performed a line bisection task in which lines were viewed under both n...

Bottom-up transfer of sensory-motor plasticity to recovery of spatial cognition: visuomotor adaptation and spatial neglect.

A large proportion of right-hemisphere stroke patients show hemispatial neglect, a neurological deficit of perception, attention, representation, and/or performing actions within their left-sided space, inducing many functional debilitating effects on everyday life, and responsible for poor functional recovery and ability to benefit from treatment. This spatial cognition disorder affects the or...

Finding Wally: prism adaptation improves visual search in chronic neglect.

Several studies have found that visuo-motor adaptation to rightward deviating prismatic goggles (prism adaptation) can alleviate symptoms of neglect after brain damage, but the long-term effect and clinical relevance of this rehabilitation approach have been questioned. In particular, the effect on visual search performance is controversial. In the present study 6 patients with chronic spatial ...

Ineffectiveness of prism adaptation on spatial neglect signs.

BACKGROUND AND PURPOSE To evaluate the effect of prism adaptation (PA) on spatial neglect signs. METHODS Ten patients (hemianopia in 6) and 8 control subjects were included. Tasks were reading single words, nonwords, and a text, bell cancellation, line bisection, and scene drawing, before and after adaptation, with either deviating or neutral prisms. RESULTS Errors were more frequent in pat...

Prism adaptation enhances activity of intact fronto-parietal areas in both hemispheres in neglect patients.

Unilateral spatial neglect involves a failure to report or orient to stimuli in the contralesional (left) space due to right brain damage, with severe handicap in everyday activities and poor rehabilitation outcome. Because behavioral studies suggest that prism adaptation may reduce spatial neglect, we investigated the neural mechanisms underlying prism effects on visuo-spatial processing in ne...
